Plan is good for one year and non-transferable. Unused benefits cannot be rolled over. Cannot be used together with other insurances. This plan is not an insurance, and does not qualify as a health plan under the Affordable Care Act. Payment is due at the time of service. Plan does not cover treatments rendered outside of this location.
